MEDICATION AT SCHOOL

 

NO medications will be administered to any student unless the following criteria have been completed: (This includes all medications, i.e. Tylenol, cough syrup, etc.)

1. A "Parent Request for District Assistance in Administering Approved Medication to a Student" form must be completed by a physician and parent.

THIS FORM MUST BE RENEWED EACH SCHOOL YEAR.

Washington State Law, SHB 2834, now requires that the necessary orders, medications, equipment, and nursing plan must be in place before a student with a life threatening condition may enroll on the first day of school.

2. Medications must be in the ORIGINAL container.

3. Office personnel will administer medications.
IT IS THE RESPONSIBILITY OF THE STUDENT TO COME TO THE OFFICE TO TAKE HIS/HER MEDICATIONS, NOT THE OFFICE PERSONNEL.

Forms for "Parent’s Request for District Assistance in Administering Medication to a Student" are available in our school office. Requests for administration of medication shall not continue beyond one (1) school year without obtaining a new written request for medication. For all students' safety, it is required that all medication be delivered to the school office instead of being sent on the bus. In an emergency, the bus driver will keep the medicine in his/her possession for the ride to school then give it to a school employee to take to the office.
